Hager Hill is a Johnson county community on Burnt Cabin Branch near its mouth on the Levisa Fork of the Big Sandy River about two miles south of Paintsville. The name probably came from the local Hager family. It was the site of Johnson Station on the Big Sandy and Kentucky River Railroad. The Hagerhill post office opened in 1903 and the name is sometimes spelled that way.

There is a trailhead for the Dawkins Line Rail Trail in Hager Hill.
